On this
page you will find 2 sets of forms. The first set is for your
information, and does not need any sort of completion. The
second set of forms includes:

release of child policy & authorization

emergency contact sheet

health history form

administration of medicine form

Permission
slips for Spruce Hills Condo Pool & PG movies

A
liability waiver/parent agreement form
We request that you print
out these forms and return them along with an immunization
record, insurance card covering the child (a copy of BOTH
SIDES), and a photo of your child (to aid staff in identifying
your child). Please do so no later than 2 weeks prior to the
week your child begins camp. You may either mail or fax us these
forms, drop them off personally, or send them in an email to theyouthcenter@embarqmail.net.
If for some reason you cannot print
out/access these forms, please contact us, and we will be happy to
provide you with a copy.
